Step 4: Waiver Forms (not online)
The Waiver Form package will be delivered to your child during the week of January 14 and must be submitted on the first day of the Club to the Program Coordinator.

*Please note Grouse Mountain Waiver and Rental Liability Waiver are not available online. They will be distributed to registered students during the week of January 13. Please bring all completed forms to the first session. Pls contact the Coordinator is your child did not receive the package by end of the day, Friday, January 17
